We believe access to healthcare and community resources begins with education, trusted relationships, and meeting people where they are. Our Community Outreach Program works directly with individuals, families, seniors, caregivers, healthcare providers, nonprofit organizations, and community partners to help connect people with resources that can improve their health and overall well-being.

Our outreach efforts are focused on reaching individuals who may have difficulty navigating healthcare coverage, understanding available benefits, or accessing the community programs designed to support them.

How We Support the Community

Through community events, educational sessions, one-on-one assistance, and strategic partnerships, we provide support that may include:

  • Medicare 101 and healthcare education
  • Medicare and Medicaid benefit education
  • Benefit coordination and benefit reviews
  • Medicaid application assistance and resource navigation
  • Prescription drug assistance resources
  • Education about available dental, vision, hearing, and supplemental benefits
  • Connections to housing and community support resources
  • Caregiver and in-home care resource education
  • Health and wellness events
  • On-site benefit education and individual consultations
  • Referrals to trusted healthcare and community partners

Community Partnerships

We understand that healthcare needs often extend beyond a doctor’s office or insurance card. Housing, transportation, food access, prescription costs, caregiving, and other social needs can all affect a person’s health and quality of life.

That’s why we believe in collaboration rather than working in isolation. We partner with healthcare practices, community organizations, senior programs, nonprofits, local businesses, and trusted professionals to connect individuals with resources that address both healthcare and everyday needs.
